Services

Lutheran Hospice services are available to patients with life-limiting illnesses who can no longer benefit from curative treatment and usually have life expectancies of six months or less, as determined by the physician. Upon admission, a Plan of Care is developed with the physician and team of hospice professionals, utilizing the patient and family's objectives for pain and symptom management.

Our Services Include:
Physician services for the medical direction of the patient's care, provided by either the patient's personal physician or the Lutheran Hospice Medical Director;
 
Regular home care visits by registered nurses to monitor the patient's condition and to provide appropriate care and maintain patient comfort;
 
Certified Nursing Assistants to assist with the patient's personal care needs, light housekeeping and brief respite periods;

 
Social work and counseling services to assess and assist with ongoing emotional needs related to death and dying, as well as assistance with community resources and services available;

Chaplain services for the patient and/or loved ones focusing on the spiritual needs of the patient and family;
 
Bereavement counseling to help patients and their loved ones with grief and loss;
 
Volunteers to support both the patients and caregivers;

Pharmacy consultation and medications for symptom control and pain relief;
 
Medical equipment and supplies;
 
Physical, Speech and occupational therapy services are available as determined by the Plan of Care;
 
Dietary Counseling.
